 Legal Obligations: Southern Medical is required by law to maintain the privacy of all your protected health information (PHI) within our organization, provide this notice to all customers, inform customers of our legal obligations, and advise customers of their additional rights concerning their protected health information.

Use and Disclosure of Protected Health Information: We will only provide your PHI to your insurance company as authorized by you to receive payment for the medical equipment you received. On rare occasions we may be required to provide your PHI if required by state or federal law and/or by court or administrative law order. We will inform you if this occurs.
